Upload Immunization Records

All students attending Coastal Carolina University are required to meet the University's immunization requirements prior to registration; a copy of your immunization record must be provided to Student Health Services prior to your enrollment. 

Immunization records must be signed and verified by a physician, physician assistant, nurse practitioner, or stamped by the health care facility. Your immunization record must include the month, day and year vaccine was given. All immunization records must be submitted in English. (These are typically the same records that students would need to submit to their high school for immunization compliance. Most students should already have possession of these records.)

Please allow 24-72 hours for your immunization records to be verified after submission.

How to Submit Immunization Documentation:

  1. Go to https://coastal.medicatconnect.com and log in with your CCU username and password.
  2. Click on the “Upload” tab towards the top of the screen
    • Select the type of document you are uploading from the “Choose document you are uploading:” drop down
    • Click “Select File” and upload your chosen immunization document.
  3. Click on the “Immunizations” tab towards the top of the screen
    • Select “Enter Dates”
    • Scroll down and enter the dates of any/all immunizations that you have received
      • If you have completed a waiver for any immunizations please select which type of waiver you have submitted from the “Waiver” drop down.
    • Once all dates are entered, click “Submit.”
  4. Click on the “Checklists and Forms” tab towards the top of the screen
    • Complete all of the forms in the *Required Immunization Documentation section
      • Health History Form
      • Meningitis Waiver Form
      • TB Risk Assessment Form
